We are not accepting medicare or medicaid clients at this time.
​
If your insurance is not listed above we are not in network and have no plans to become in network with that particular plan. We can provide a superbill/invoice for you to submit to insurance for out of network reimbursements.
Cash rates are available upon request for those without insurance, those who do not want to use their in network plan, or those who have out of network insurance plans.

We also accept credit cards and HSA cards for payment.
Forms and paperwork
All forms are completed during scheduled appointments. This includes disability, accommodations, ESA, and other letters.
No show and Late cancellations
All appointments must be cancelled within 2 business days prior to the scheduled visit. No shows and late cancellations will be charged $150.00 for the missed visit.
